Please complete the warranty claim form. You may need to provide us with further proof of purchase. In order to fill out this form, please have the following information handy:

• approximate date of purchase
• the UPC code from your product label
• location you purchased the product from

A shipping and handling fee of $20.00 will be charged for each warranty replacement mat.   Upon approval of your claim, a customer service representative will contact you to arrange payment of shipping charges, via MasterCharge, VISA or Discover. We cannot accept checks.
