Please complete the warranty claim form. You may need to provide us with further proof of purchase. In order to fill out this form, please have the following information handy:

• approximate date of purchase
• the UPC code from your product label
• location you purchased the product from

Shipping charges will apply to most warranty replacements.  Upon approval of your claim, a customer service representative will contact you to arrange payment of shipping charges.

In order for us to establish a credit account in your name, we request that you fill out the following Credit Authorization Form.
